If you are unable to fill out the online
questionnaire with your browser, please mail in your completed questionnaire
according to the instructions on your summons.
Please note, all the information you submit using this system remains private
and confidential. It is used exclusively by the Berks County Courts for
the sole purpose of jury selection.
